AltaMed Health Services has been providing quality health care to the underserved and uninsured in Southern California for more than 35 years. It has long been our goal to be a beacon of hope for minorities and economically disadvantaged communities. Altamed Health Services Corporation provides free primary health care to public housing residents of Ramona Gardens, William Mead, Estrada Courts as well as non-public housing uninsured residents.

Primary health care including adult medicine, pediatrics, perinatal, OB/GYN, Women's care, complete physical exams, Tuberculosis testing, Diabetes (accuchecks), X-ray, ultrasound, mammography, laboratory, lead testing, drug testing; health education, pharmacy, optometry, and dental services (Patients are referred to the Highland Park clinic).

Project Description
The PHPC Program provides residents of selected public housing communities access to comprehensive, consistent health care services through the direct provision of primary health care services, health promotion, and disease prevention activities on or near the premises of public housing developments. The Program provides primary health care and dental services through the Mobile Medical Unit (MMU).

The MMU makes weekly and bi-weekly visits to designated public housing sites and works in conjunction with outreach workers and a dual diagnosis counselor in service provision. Outreach workers promote program services to residents and offer health education and assistance in completing forms/paperwork as needed, while the dual diagnosis counselor provides mental health and substance abuse counseling and referrals to residents in their homes. Additionally, residents referred for health care services not provided by PHPC in their housing community will be provided access to transportation.

Services
The PHPC Program provides primary health care services to children and adults that include preventative dental care, health education, outreach, diagnostic, laboratory, case management and mental health services. The Program refers residents to qualified facilities and practitioners for other necessary services, including pharmacy, x-ray, substance abuse and mental health services. The Program helps residents to establish eligibility for assistance under programs such as Medi-Cal and Healthy Families (low-cost health insurance for children).

The PHPC Program is a collaborative effort between Family Health Centers of San Diego and the San Diego Housing Commission, which effectively links areas of agency expertise.

Project Description
La Maestra Family Clinic (LMFC) is a not-for-profit, primary care community health center founded in July 1991 to serve the poor, immigrants, and refugees. La Maestra means "The Teacher" and the health center is unique in that it provides healthcare services, dental and mental health, as well as job training and placement, educational and social services and eligibility assistance. More than 19 languages are spoken by the LMFC staff.

LMFC uses a cultural liaison model to help recruit and train individuals from ethnically diverse populations to become medical assistants and support staff as medically trained cultural liaisons. This model provides a two-way conduit for communication between the provider and the patient, educating both sides about the other.

Services
In addition to providing medical and dental services, LMFC has also developed other enabling services necessary to the well-being and health of the family: job training, citizenship assistance, health and housing, mentoring for ethnic CBO's, linkages to social services, and advocacy for cultural and linguistic competence in healthcare, among others.

Project Description
The Los Angeles Christian Health Centers is a non-profit Federally Qualified Health Center providing quality, comprehensive medical care to public housing residents and other underserved individuals in the heart of downtown Los Angeles. Their programs are designed to facilitate holistic restoration of individuals and families whose lives have been devastated by physical illness, substance abuse, economic hardship, and psychiatric diseases.

Primary and Preventive medical care for adults and children, dental care and screenings, chiropractic care, well child care and immunizations, TB screening and follow-up therapy, HIV/STD education, counseling and testing, on-site dispensary, social work and case management, substance abuse and mental health assessments and counseling, outreach in streets and shelters--including primary care services and medications provided at multiple locations in Skid Row, general health education (both for individuals and groups), and referrals to specialty providers and other support services.

The San Ysidro PHPC program, Healthy Connection, is a mobile health care program that offers comprehensive primary and preventative care services to the residents of 10 public housing developments, nearly 4,000 residents. A medical team routinely visits each development to conduct outreach, educational activities and screenings. SYHC provides case management and transportation assistance to residents.

Project Description
The Westside of Los Angeles is home to many people with low incomes and no health insurance. Mar Vista Gardens Housing Development in Culver City is home to many of the poorest residents in the West Health District. Seventy-seven percent are Latinos; and, for the most part, Spanish is their primary language. Patient demographics include: nearly 50 percent live below the federal poverty level, twenty-five percent of households earn less than $10,000 per year, 28 percent of households are headed by single mothers, about 10 percent of the adult population is unemployed, and one quarter is disabled.

In May 2003, Venice Family Clinic and Westside Family Health Center began providing on-site primary care to residents of Mar Vista Gardens and the surrounding area. The Mar Vista Gardens Clinic will improve access to primary care for about 1,000 poor and uninsured men, women and children in 4,000 annual patient visits.

Primary care, chronic disease care and management for diabetes, hypertension, heart disease, asthma and depression, comprehensive pediatric care, including well-child care, immunizations, acute and chronic care, comprehensive health care for teens, comprehensive women's reproductive health care, direct care and referrals for homeless persons living near the housing project, urgent care through telephone triage with an RN, mental health and domestic violence services, through on-call social worker and by referral to VFC's other clinical sites, referrals for dental care through the UCLA Venice Dental Center for adults and children, health education, promotion and disease prevention, by referral and/or on-site health educator, health insurance outreach and enrollment into low/no cost programs, children First Early Child Development Program, providing home-based child development services, education, case management and child care, referrals for specialty care.
